To join one of our Patient and Family Advisory Councils, you must complete an application and go through an interview. Space is limited so it is possible that not all applicants will be able to serve on the Council, but all applications will be kept on file as future openings occur. Application Process Requirements/Steps:

  • PFAC/Volunteer Services Application submission (submit below)
  • Interview with PFAC Executive Team
  • Once approved by PFAC leadership, Volunteer Services Review will require:
  • Copy of Driver’s License or government ID
  • Completed forms (provided by Volunteer Services):
  • Background check form
  • HIPAA Acknowledgement form
  • Conflict of Interest Form
  • Online Orientation - link provided to applicant by Volunteer Services
