Dr. Peter Chang of Tampa General Hospital is Recognized as a Chief Transformation Officer to Know

The academic health system’s Senior Vice President and Chief Transformation Officer is recognized by Becker’s Hospital Review for reimagining digital experience and care delivery models at Tampa General.

 

Tampa, FL (Sept. 10, 2024) – Dr. Peter Chang, senior vice president and chief transformation officer at Tampa General Hospital (TGH), has been recognized by Becker’s Hospital Review as part of its 2024 class of “48 Chief Transformation Officers to Know.” This list celebrates the achievements of chief transformation officers at leading hospitals and health systems nationwide, who have contributed to creating meaningful change within their organizations, including enhancing the digital experience and restructuring care delivery models. Chang is one of only four Florida-based chief transformation officers recognized by Becker’s on this prestigious list.

In his capacity as chief transformation officer, Chang designs forward-looking health care strategies to improve clinical quality, efficiency and care coordination while decreasing the total cost of care at Tampa General. His work is focused on scaling care coordination systemwide to provide personalized health care navigation to every patient, using technology to optimize care delivery and make health care more accessible and efficient.

To advance these strategic priorities, Chang led the design and implementation of Tampa General’s “Hospital at Home” program (TGH at Home). Prior to his role in design, he created and operated CareComm, Tampa General’s Clinical Command Center, focused on system-wide efficiency and quality of care.

Under Chang’s leadership, Tampa General consistently invests in strategic partnerships, developing platforms that put patients at the center of their care. His continued work to scale and improve the TGH at Home program has already resulted in reduced readmissions, lower cost of care, exceptional outcomes, increased patient satisfaction and more options for patients in the Tampa Bay community and beyond. Specifically, the TGH at Home program has achieved readmission rates of approximately 4-5 percent, as compared to 17-18 percent for inpatients at the brick-and-mortar facility, and patient experience scores are 30-40 percent higher for the TGH at Home program than for inpatient care units.

TGH at Home has made it possible for patients to recover in the comfort of their own homes and familiar surroundings without compromising the quality of care they receive, continuing to meet the same standards of acute hospital care. Since its launch in 2022, patients enrolled in the TGH at Home program have not experienced any pressure injuries or contracted hospital-acquired infections. The program has served nearly 700 patients and is on track to achieve an average daily census of approximately 20 patients by the end of 2024.

Because of Chang’s work building and integrating CareComm, Tampa General has improved care coordination through better capacity management, reduced average length of stay, lives saved by early warning systems, such as for timely sepsis diagnosis and management, and has realized significant cost savings while delivering lower readmissions, lower cost of care, improved outcomes and increased patient satisfaction. He also led designing and integrating a specialized command center to monitor census, quality and outcomes for the TGH at Home program. In addition to his success with TGH at Home and CareComm, he has further enhanced care coordination and access by creating a “patient experience center” to make scheduling and management of appointments more seamless for patients, as well as the academic health system’s providers and team members.

Collectively, Chang’s work reduces friction points for Tampa General’s patients, providers, and team members, improving accessibility to and affordability of world-class patient care. He has also created novel strategies to increase patient engagement, raising the bar for the industry and redefining how care is delivered. His contributions and commitment to pioneering the future of care coordination have cemented Tampa General as an innovator shaping the academic and health care landscapes, leading the industry in Florida and nationally.
